MSU Youth Programs Pick-Up, Drop-Off, Commuter Form
As an additional safety measure for children participating in MSU youth programs, such as CMS, MSU has implemented a youth program policy which requires parents/guardians of children under the age of 18 to complete the MSU Youth Programs Pick-Up, Drop-Off, and Commuter Permission Form.
Required for all MSU youth programs, this new policy offers an added level of security for children under the age of 18 by providing:
- list of all individuals (including parents/guardians) who are authorized to drop-off or pick-up the child; and/or
- permission for youth to commute independently to and from the youth program (i.e. drive, walk, bike, ride CATA, etc.).
Tips for Completing This Form for CMS Students/Clients:
- Participant Name and Program: please list the full name and CMS program(s) in which the student/client is currently enrolled. An individual form needs to be completed for each student/client.
- Permission for Early/Alternative Release: in this section, please list all individuals (including the parents/guardians) who are authorized to drop-off or pick-up the students/clients. If at any point, a person needs to be added or removed from this list, please contact CMS.
- Permission for Youth Participant to Commute Independently: if the child/client commutes to and from CMS independently (i.e. drives, walks, bikes, rides CATA, etc.), please complete this section.
Completed and signed forms can be dropped off at the CMS office or emailed (as a PDF) to commusic@msu.edu.
